0215(3) Licensor observed a child with a medical condition (siezures) did not have a medication authorization form signed by the parent. 0215 (3)(a)(i) Licensor observed a child with an Epi-Pen did not have the child's first name and last name on the life saving medication (Epi-pen was not stored in the original prescription box). Licensor observed a child with a health condition had a life saving seizure medication that was expired as of 09/24/2024. 0215 (3) Licensor observed the expired life saving seizure medication was not returned to the parent for a refill.
